Lifestyle-related disease management fee (I)/(II)

This program is for patients who have agreed to receive medical guidance regarding hypertension, dyslipidemia, and diabetes.

As part of measures to combat the increasing number of lifestyle-related diseases, the Ministry of Health, Labour and Welfare has instructed that on June 1, 2024, it will revise medical fees, abolish the "specific disease management fee" that has been calculated by clinics until now, and switch to a "lifestyle-related disease management fee" that provides more specialized and comprehensive treatment management based on individual treatment plans.

In line with this revision, from June 1, 2024, in accordance with the guidelines of the Ministry of Health, Labour and Welfare, patients whose main illness is hypertension, dyslipidemia, or diabetes and who have been billed for the "specific disease management fee" will be switched to the "lifestyle-related disease management fee."

As a result of this revision, we will be requiring patients to sign a "Treatment Plan" that includes individual goal setting, specific instructions regarding blood pressure, weight, diet, and exercise, and test results. We appreciate your cooperation in this matter.

Depending on the patient's condition, and at the doctor's discretion, refills may be prescribed or medication may be prescribed for longer periods of time than 28 days.

*First-time patients and those requiring blood tests should arrive 30 minutes before the end of morning and afternoon clinic hours.

*If you have an appointment for a gastroscopy or abdominal ultrasound between 8:30 and 9:00, that appointment will be given priority.

* Except for scheduled examinations, patients will generally be seen in the order in which they arrive.
